Overview: We are seeking a compassionate and dedicated individual to join our team as a Specialist Family Support Worker. In this role, you will be the key worker for an adult with Down syndrome, providing personalised support to facilitate reconnection with their family. Your primary responsibility will be to act as a liaison person, ensuring effective communication between the client and their family. You will need good communication skills, an empathetic approach and a strong commitment to enhancing the well-being of the individual with Down syndrome to support the fostering of positive relationships between the client and their family members.
Key Responsibilities:
Client Advocacy - Serve as the primary point of contact and advocate for the adult with Down syndrome.
Spend time with individual to understand the unique needs, preferences, and goals of the client.
Develop and implement personalised support plans to aid in the reunification process.
Communication Facilitation - act as liaison for the client and their family, ensuring clear and effective communication.
Relay important information, updates, and concerns between the client and family members.
Foster an environment of open and respectful dialogue to enhance understanding and collaboration.
Collaborate with external agencies, healthcare professionals, and community resources to provide holistic support to the client.
Facilitate family meetings and discussions to address any challenges and celebrate successes.
Provide emotional support to both the client and their family throughout the reunification process.
Assist in managing expectations and addressing any emotional or psychological challenges that may arise.
Documentation and Reporting - Maintain accurate and confidential records of client progress, interactions, and support plans.
Generate regular reports to track milestones, challenges, and improvements in the reunification process.
Qualifications and Skills:
Bachelor's degree or alternative qualification in a related field.
Previous experience working with individuals with Down syndrome or developmental disabilities.
Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals and families.
Training will be provided including Supporting People with Down Syndrome, Communication Support and De-Escalation.
Empathy, patience, and a commitment to promoting the independence and well-being of adults with Down syndrome.
Ability to work independently and collaboratively in a team environment.
